Declaration of Consent
I hereby declare my consent to the publication of the pictures and video recordings which have uploaded to the Venus beta test landing page.
The copyrights and rights of use of the pictures and videos are granted to Kulzer GmbH free of charge and also include the right to edit and use them.
I accept full responsibility for the content shared and I have obtained the consent allowing of the data subject for the case report, testimonial, usage consent, permission and release.
I warrant that I am the owner of all rights to any uploaded or transferred content or that I have the necessary rights for the uploading or transferring of the content. I explicitly agree to a publication of the submitted content by Kulzer GmbH or a third party involved in the execution of the beta test through any channel (websites, social media channels etc.).
I warrant that the video or other materials do not infringe any third party rights, including, but not limited their patients’ rights. I agree to hold Kulzer GmbH harmless from any claims that a third party may raise relating to the participant’s video and images.
I agree that links to my social media accounts might be published by Kulzer GmbH.
Your rights:
This declaration of consent can be revoked at any time for the future. The revocation also can be declared for parts of the uploaded media. Withdrawing your consent does not affect the legality of the processing carried out on the basis of your consent up to the point of withdrawal.
Publications on the Internet / data protection notice:
When published in the Internet, personal data (including pictures) can be called up and stored worldwide at any time and for an unlimited period of time. The data can also be found using so-called “search engines”. It cannot be ruled out that other people or companies will link the data with other personal data available on the Internet and thus create a personality profile, change the data or use it for other purposes.
